The scariest bugs in my AI memory system don’t throw errors.
They just quietly give you wrong answers.
The Index Problem
MEMORY.md is my agent’s index. It points to files. Topics, context, rules. The whole system depends on it being accurate.
Except sometimes the files don’t exist anymore.
Agent reads the index, finds a pointer, trusts it, acts on it. The file is gone. It doesn’t know. No error. Just stale data dressed up as current truth.
Stale Memory
Worse: the agent remembered things that were no longer true.
I had Odin listed as Sonnet in the memory files for a few days after switching to Grok. So every session it was working from the wrong model context. Confidently. Completely wrong.
Stale memory isn’t a minor bug. It’s a ghost that gives you directions.
The Cooldown Bug
Here’s one that cost me half a day.
OpenClaw hit a billing error and stored a cooldown state in a file. I restarted the agent thinking that would clear it.
Restarts clear memory. Not files.
Agent came back up, read the file, stayed in cooldown mode. Sat dead for hours while I tried to figure out what was wrong.
Context Compression
Long conversations have a similar problem in the other direction. Early context gets compressed out.
You told the agent something important 30 minutes ago. It doesn’t know that anymore. It’ll act like you never said it.
And it won’t tell you it forgot.
The Fix
Fix that actually worked: a daily cron that validates every pointer in MEMORY.md. Checks if the file exists. Flags broken links. Runs at 5am before anything else touches the system.
Boring script. Ten lines. Keeps the whole thing honest.
The Lesson
AI memory doesn’t fail loudly. It fails quietly, in the background, while you trust it.
You don’t get a stack trace. You get a confident wrong answer.
That’s the bug nobody talks about.
